Job Description
Family Medicine APP (experienced) - Rural Health Clinic in Columbia, NH
We are seeking an experienced Family Medicine Advanced Practice Provider (APP) to join our rural health clinic in Columbia, NH.
Key Responsibilities:

• Provide high-quality primary care services to patients of all ages
• Perform comprehensive physical examinations and assessments
• Diagnose and treat common acute and chronic illnesses
• Order and interpret diagnostic tests
• Develop and implement treatment plans
• Counsel patients on preventive healthcare and lifestyle modifications
• Collaborate with physicians and other healthcare professionals to ensure coordinated patient care

Qualifications:

• Current certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) with experience in family medicine
• Active state license and DEA registration
• Minimum of 2 years of experience working in a primary care or family medicine setting
• Strong clinical skills and ability to work independently
• Excellent communication and interpersonal skills
• Comfort working in a rural healthcare setting
